What do Systems of Bias look like in America Today?

According to the U.S. Sentencing Commission, in the federal justice system, African American men on average receive a sentence term that is 10% longer than their white counterparts receive for the same criminal offences. This disproportionate punishment can also be found in other systems across America. As reported by the Department of Education for the 2009-2010 school year, black students were found to be 3.5 times more likely to be expelled than their white peers. 

Dialogue surrounding differential treatment in America has been brought to light by individuals like Phillip Agnuew of the Dream Defenders. What are the root causes of these biases within American systems?  How do we overcome them?
